Our copay grants offer funds to cover your out-of-pocket medication costs. Through our copay grants, we offer a fast, reliable way to get help paying for medications.

About health insurance premium grants

PAN’s health insurance premium grants help pay for health insurance premiums for patients being treated for various disease states.

About transportation grants

Need help with transportation costs related to your overall health? You can get this extra transportation benefit if you have an existing PAN grant.

How to apply for grants

How wait lists work

If your fund is closed, and you are eligible for the fund, you can sign up for the wait list. When the fund opens, we’ll contact you right away. Learn how our wait lists work.

After you apply: verify your income or premium

Here are the steps to verify your income or provide documentation of your health insurance premium to complete the application process.

Using and managing grants

It’s easy to start using your PAN grant right away. We explain what’s covered, where you can use the funds, using transportation debit cards, how to check your balance, and how to keep your grant active.

Claims and reimbursement overview

Learn how to submit reimbursements or file claims whether you’re a patient, provider, or pharmacy. We help you understand the process and how to receive payments. You’ll find links to provider and pharmacy claims and billing guides, as well as to the patient direct member reimbursement form (DMR form).

Apply for additional funding or renew a grant

Your initial PAN grant provides you with funding for 12 months. You can apply for additional funding if those funds run out or apply for a renewal grant when your initial grant period is ending or has ended.
