Frequently asked questions about our transportation fund 

Here are answers to questions we often get from patients about our transportation fund. 

Note: healthcare professionals cannot apply for transportation assistance on behalf of patients. Patients or their caregiver must apply directly. If you are a healthcare professional, we encourage to check your patient’s eligibility and refer them

About the transportation fund

  • Our transportation fund helps you pay for transportation to and from activities that improve overall health.  

    After your application is approved, you will get a $500 prepaid Visa debit card in the mail within 15-20 business days. You can use the card to pay for covered expenses, but you must activate your card before you can use it. 

     

  • To be eligible, you must meet all the following requirements: 

    • Be currently enrolled in a PAN co-pay or premium disease fund and getting treatment for that disease. 
    • Live and get treatment in the United States or U.S. territories. (U.S. citizenship is not needed.) 
    • Have health insurance that covers your qualifying medication or product. 
    • Have an income that falls at or below 500% of the federal poverty level.

    Note: you are not eligible for a transportation grant if you are on a disease fund wait list or have an expired or inactive PAN grant. Check your enrollment status on the online patient portal. 

  • If you are not eligible but still need financial support for your transportation, review our transportation resources directory, which includes referrals to organizations that may be able to help. 

    We also have a directory of non-emergency medication transportation services you can reference for organizations that offer healthcare transportation needs. 

  • You can use your Visa prepaid debit card to pay for: 

    • Airfare (tickets, fees) 
    • Car rental (car, insurance)  
    • Gas (only at the fuel pump, cannot be used to pre-pay inside) 
    • Lodging, such as hotels or temporary housing 
    • Non-emergency medical transportation (shuttle service) 
    • Parking (lot, meter fees) 
    • Public transportation fares (bus, metro, train) 
    • Rideshare (app payments, fees) 
    • Taxi (fare) 
    • Tolls (electronic payment)  

    Note: we monitor your card activity and will cancel your card if it is used for any unauthorized purchases. We also have the right to cancel your grant if a transaction has not been made on your card in 120 days or if there is a change in your eligibility. 

  • This fund does not cover non-transportation related goods or services, including gift cards, mileage reimbursement, food/groceries, or concierge services. 

    Be advised that all card activity is actively monitored. The purchase of unauthorized items, like gift cards, is considered fraud and will result in the immediate loss of the grant and card balance. 

  • The quickest way to add yourself to the wait list is to visit the fund page on our website.

Apply for and manage your grant

  • The quickest way to apply is to visit the fund page on our website.  

    You can also use your portal account or call us at 1-866-316-7263 from Monday through Friday, 9:00 a.m. to 5:30 p.m. ET. 

  • If your application has been conditionally approved, we need to verify your income before your card can be activated. You will need to send us your most recent tax return, along with your name and ID number.  

    There are two ways you can submit this information:  

    • Use this online form to submit your documents.  
    • Fax your documents to 1-866-316-7261.  

    If you do not file taxes, call us at 1-866-316-7263 from Monday through Friday, 9:00 a.m. to 5:30 p.m. ET.  

  • To keep your grant active, you must use your prepaid debit card to make a purchase for an eligible expense every 120 days during your enrollment period. 

    If you only need transportation once or twice a year and you get an alert from us indicating that you must use your grant soon, contact us 

  • You can check your grant balance by logging in to the online portal or calling us at 1-866-316-7263 from Monday through Friday, 9:00 a.m. to 5:30 p.m. ET.  

  • Yes. You may apply to renew your grant if you still meet the fund’s eligibility criteria and your grant period has ended or is about to end.  

     You can apply up to 30 days before the current grant period ends, even if there is a grant balance remaining. If a renewal grant is approved, a new 12-month eligibility period begins. 

Use your debit card

  • Submit a card activation request here. We will email you confirmation that your card has been activated. Typically, this process takes one business day or less. 

  • Cards should be swiped using the credit card option. No transaction fees or pin numbers are required.  

    The funds are withdrawn from your account immediately after the card is swiped and you can check your grant balance by logging in to your PAN portal or calling us.  

  • Your total daily spending limit is $350. There are also daily limits by category, listed below.  

    Daily limits:  

    • Airfare: $300 
    • Car rental: $100 
    • Gas: $75 ($150 limit per week) 
    • Hotel: $300 
    • Parking: $50 
    • Rideshare: $100 
    • Taxi, bus, or train fare: $100 

    Your card will be declined if you go over a spending limit without permission. Remember: we can increase limits for individuals with special circumstances.

  • Visa cards are often accepted by businesses, medical offices, pharmacies, and more.

    Note: your debit card may only be used for the covered services within the transportation assistance program.  

  • No, you do not need pre-approval to make a purchase.  

  • While we do not require you to submit your receipts, we recommend saving them in case of an audit. 

    Be advised that all card activity is actively monitored, and we can cancel your grant if the card is used for any unintended or inappropriate purposes. 

  • Your grant lasts for 12 months but you must use your prepaid debit card to make a purchase for an eligible expense every 120 days during your enrollment period. 

    If you only need transportation once or twice a year and you get an alert from us indicating that you must use your grant soon, contact us 

Troubleshoot issues

  • You should receive a Visa prepaid debit card in the mail 15-20 business days after your grant is approved. If you have not received your card after 25 business days, contact us for further assistance. 

  • Note that your card is restricted to the expenses covered under the fund.  

    If a merchant you want to use is not available or if your card is declined for a covered expense, give us a call at 1-866-316-7263 from Monday through Friday, 9:00 a.m. to 5:30 p.m. ET. 

  • If you lose your debit card, contact us at 1-866-316-7263, Monday through Friday, 9:00 a.m. to 5:30 p.m. ET.
