The following is a list of documents that you may need to access to submit an application for Medicare’s Extra Help program, also known as the Low-Income Subsidy program (LIS). You may not have, or need, some of these documents. Check off each item as you obtain it and ensure that you are accurately reflecting your income and assets on your application. If you need help obtaining any of these documents or have questions about their use, contact your local State Health Insurance Assistance Program (SHIP) office.
